Horizons Team

Aberdeen City Health and Social Care Partnership

Who are we?

We are a multidisciplinary team, comprising O.T, physio, dietetic and SLT. Our team aims to provide a high quality integrated service to clients who have complex neurological conditions.

Who Do We Work With?

We work with adults age 16 and over who present with problems relating to stroke, head injury, cerebral palsy, functional neurological disorder, and progressive neurological conditions (e.g. Parkinson’s, Motor Neurone Disease, Multiple Sclerosis). The Speech and Language therapist works with symptoms related to these conditions: problems speaking clearly (dysarthria or dyspraxia), understanding speech or written words (aphasia), expressing themselves through speech or writing (aphasia), eating/drinking and swallowing problems (dysphagia).

What to expect when referred

> We will contact the client initially, where possible, by phone to discuss the communication or swallowing problem. This is part of the triage process to determine whether any further appointments are required.

> We will assess the client’s communication or swallowing needs over the phone/ via a video call / in person.

> Depending on the need we might offer a single appointment and advice or we might offer a block of therapy sessions. Where possible we will attempt to co-ordinate these with appointments from other disciplines e.g. Physio/ OT.

> If therapy is appropriate we will discuss and agree goals with the client.
