Information for people who work in a Nursing Home or Care Home

Many of our clients live in Nursing Homes or Care Homes. Care Home residents can be referred for swallowing or communication assessment support by Care or Nursing Homes.

Care staff will have the appropriate forms to complete. Liaising closely with care staff is important to us and we value all input. We have included some information here that you might find helpful. If you can’t find what you are looking for or would like to discuss a referral or an existing client please call our Duty Phone Line: 01224 558377.

Once referred SLT staff phone you to assess need and find out more information. For most referrals a visit will then be arranged. The resident will be assessed and information gained from the person, the staff and their relatives. After assessment recommendations and/or therapy will take place.

Training for Care Homes

A Care Home training project is currently being undertaken. Staff from care homes are receiving training and support around Eating, drinking and swallowing. Training meets the requirements of the SSSC and focuses in on identifying eating drinking and swallowing difficulties, identifying issues that may contribute to difficulties, problem solving and strategies to support residents. The aim is to reinforce person -centered care approaches.
 
A new referral form is introduced at the end of training and ongoing support for the use of this and implementation of training is also provided.
